public with greater convenience in electronic submissions to the
Government. Apart from downloading conventional forms and submitting
them in paper form, you may now choose to download the electronic form
which facilitates you to fill-in and submit Government forms
electronically. Please visit

about the "e-Form".
Electronic Forms (e-Form) that can be installed in a PC for
off-line completion. To operate the e-Form, you have to download and
install the e-Form core program that is available at
http://www.info.gov.hk/digital21/eform/index.html. Submission of the
completed e-Form can be sent as attachment to e-mail to the relevant
e-mail accounts.

| Notice of Intention to Object to
Bankrupt's Discharge under Section 30A(4) of the Bankruptcy
Ordinance (Chapter 6) This form
(Form 82) should be used by creditors who wish to object to a
bankrupt's discharge. Creditors should refer to the relevant
"Notice To Creditors by Trustee" and file the completed Form 82
to:-
- the Court of First Instance at 38, Queensway, Hong
Kong; and

Request for Search of
Compulsory Companies Winding-up/Bankruptcy Records, ( )
This form is used by the public to check if a person or a
company is involved in insolvency proceedings.
Applicant who wishes to conduct a
compulsory winding-up search is required to complete the
upper part of the form. The lower part of the form should be
filled in for a bankruptcy search.
Trading name of the company or Identity
Card no. of the search target should be given as far as
practicable so that more accurate search result can be
provided
A search fee of HKD85.00 is payable for
each search request. |

Proof of Debt
(Winding-up), ( )
This is a document by which a creditor seeks to
establish his claim against a company in liquidation.
In accordance with Rule 79 of the
Companies (Winding-up) Rules, every creditor in a winding up
by the court shall prove his debt, unless a judge in any
particular winding up shall give directions that any
creditors or classes of creditors shall be admitted without
proof. .
A debt may be proved in a compulsory
winding up by delivering or sending through the post to the
Official Receiver this proof of debt form and accompanied by
a filing fee of HKD40.00. No filing fee will be payable for
any claim of debt not exceeding HKD250.00 or for employees' claim.
A proof of debt may be made by the
creditor himself or by a person authorized by or on behalf
of the creditor and having knowledge of the facts.
All items in the form should be filled
up. Original supporting documents should be submitted
together with the proof. |

Residential/ Correspondence Address (

The form is used by a creditor as a notification to the Official
Receiver of the change of his residential or correspondence
address after filing of a proof of debt. Upon receipt of this
form, the Official Receiver will update his record and send
future correspondence to the creditor at the new address. |

Authority to Liquidator/ Trustee to
Pay Dividends to Another Person, ( )
If a creditor to whom dividends are payable desires that they
shall be paid to some other person he may lodge with the
Official Receiver a document in this form which shall be a
sufficient authority for payment of the dividend to the person
therein named.
This authority is to remain in force
until revoked by the creditor in writing. |
